A little assistance
_
A list of things to bring with you for your hospitalisation has been developed in order to help you not to forget anything.
For your stay
For administrative purposes, remember to bring:
- The yellow questionnaire for the anaesthetic
- The administrative questionnaire
- The consent form for the anaesthetist
- A copy of your insurance contract(s)
- An official identification document: identity card, passport, driving licence or resident permit
Don’t forget to bring:
- Any medicine that you usually take (if possible in its original packaging)
- Any x-rays that may be necessary
- Your medical documents
- Your personal effects (toiletries, books, slippers...)
For your comfort, we provide you with:
- A toilet bag with the basic necessities
- A pair of slippers
- A robe
- A “cuddle” cushion
